Safeguarding and incident response

People have the right to feel safe, respected and heard. Safeguarding processes should help staff respond quickly, record facts clearly and escalate concerns appropriately.

Incident reporting

Record when it happened, where it happened, who was present, witnesses, what happened before, objective facts and immediate actions.

Manager review

Managers should review priority, actions, notification needs, external reporting decisions and closure evidence.

Participant voice

Where safe and appropriate, the participant’s view and preferred outcome should be recorded and respected.

Restrictive practices

Any restrictive practice concern should be escalated according to approved behaviour support and regulatory requirements.

Evidence and documents

Evidence should be stored securely with restricted access and clear review history.

Learning and prevention

Corrective actions, staff training and support plan changes should be tracked to reduce future risk.
