Due Care Services collects information to respond to enquiries, review referrals, plan safe supports and communicate with authorised people.

What information may be collected

Contact details, referral information, NDIS plan details, support needs, consent preferences, health and safety information, worker compliance records, service notes and billing readiness information.

Why it is collected

To respond to referrals, plan services, match supports, manage safety, keep records, communicate with authorised people, meet legal or funding obligations, and improve service quality.

Who may see information

Only authorised people should access information: the participant, approved representatives, care team members, managers, billing/admin staff, clinicians or regulators where required by law.

How information is protected

Due Care Services aims to use secure login, role-based access, private document storage, staff training, consent records and careful sharing rules.

Participant rights

Participants should be able to ask what information is held, request corrections, update consent, ask questions about sharing, and make a privacy concern or complaint.
