Community access support in Melton and Melbourne’s west
Community access support helps participants get out, connect with others, attend activities and build confidence outside the home.
Community access support may include appointments, shopping, social activities, recreation, community programs, transport-related support and skill-building linked to the participant’s goals.
What is community access support?
Community access support is about helping a participant participate in everyday life outside the home. This may include going to appointments, visiting local places, joining activities, practising travel skills, meeting people or building confidence in the community.
For some people, the goal is independence. For others, the goal may be safety, routine, social connection or reducing isolation.

Common questions
Is community access only for social outings?
No. Social connection is important, but community access can also include appointments, errands, daily tasks, travel training, confidence-building and activities linked to NDIS goals.
Can support be one-to-one?
Many participants receive one-to-one support, depending on their plan, needs and service agreement. The right support level should be discussed during referral and intake.
What should I include in a community access referral?
Include the participant’s goals, preferred activities, transport needs, safety concerns, communication needs, preferred days and any places they want to attend.
Due Care can review the participant’s goals and help plan safe, practical community access support.
